California Community Wholeness and Enrichment RFP

San Bernardino County Department of Behavioral Health is seeking proposals from interested and qualified organizations and agencies to provide a proposed plan for providing Prevention and Early Intervention Community Wholeness and Enrichment services.

Scope of Work

The targeted population for this program is transitional age youth (TAY) (ages 16-25) and adults (ages 26-59), and their families through the use of community services throughout all regions of San Bernardino County. The CWE program includes strategies that will help create access and linkage to treatment, improve timely access to Behavioral Health services for participants and families from underserved populations, and reduce and circumvent stigma, including self-stigma and discrimination. The CWE program also engages, encourages, educates, and/or trains potential responders about ways to recognize and respond effectively to early signs of potentially severe and disabling mental illness through various outreach activities.

All Proposers must:

Be a non-profit, for-profit organization or other legally constituted business entity.
Have a current Medi-Cal Certification or have the ability to become Medi-Cal Certified, if applicable.
Have no record of unsatisfactory performance with any government agency or County Department including DBH in the twenty-four (24) month period immediately preceding the date of issuance of this RFP. Proposers who are or have been seriously deficient in contract performance during this period, in the absence of circumstances properly beyond the control of the Proposer, shall be presumed to be unable to meet this requirement.
