Speech Language Pathologist Services RFP
MVESD#16 is seeking sealed proposals for Speech Language Pathologist Services, RFP # 26-01-2030 and Physical Therapist Services, RFP # 26-02-2030.
- MVESD16 will fulfill the following requirements and perform the following services in a satisfactory and proper manner. MVESD16 shall:
- Generate a list of approved Speech Language Pathologists assigned to the District and the maximum number of hours each is allowed to bill weekly
- Provide a pre-service orientation meeting for contracting Speech Language Pathologists to address procedures related to licensure documentation, fingerprinting and background checks, district-required timesheets, service documentation, and Federal, State and District requirements
- Provide Speech Language Pathology Assistants
- Provide a district-approved timesheet to be used by all contracted Speech Language Pathologists
- Provide a list of students who have been referred for evaluation or identified by the IEP committee to receive Speech Language Service to each service provider
- Provide, for each student referred, referral information; case histories, including educational, family and medical information, hearing and vision screening: completed educational diagnostic and ancillary evaluation reports; IEP; and other relevant data necessary to design and implement Speech Language
- Provide reasonable materials for use by the Contractor when needed as determined by District
